Home Health Aide, Personal Care Giver
- Assists the patient with personal care, ambulation and exercise under professional supervision
Physical Therapy
- Provides therapeutic exercises related to a specific loss of function
- Provides training for safe utilization of adaptive equipment and assistive device for Activities of Daily Living (ADL)
Speech Therapy
- Assists the patient with swallowing difficulties and regaining speech, which may have been impaired following illness or accident
Occupational Therapy
- Assists the patient in developing wholesome activities and skills, and facilitate maximum self care to enable patients to return to former level of function
Medical Social Service
Assists the patient in dealing with social and emotional factors related to illness by utilizing community resources. Services may include:
- Education regarding alternative living arrangement
- Advance directives (Power of Attorney for Health Care or Living Will) assistance
- Community resource planning and coordination
- Crisis intervention
- Financial assessment
- Counseling with life-style changes
- Teaching regarding coping with loss and change
Home Healthcare Programs
It is vital to individualize the needs of each patient and the programs are guidelines for a baseline approach to each disease process. Some of the programs include but are not limited to:
- Congestive Heart Failure Management
